It's mostly tried and true methods and design from previous successful construction. Maybe some change was not integrated correctly, but it's too soon to guess. I'd lean toward bad fasteners or an unexpected change in material strengths somewhere. The guys that buck rivets in large beam constructions are not amateurs. They generally work as a team that has experience. I'm sure that will be checked also. The temperature of the heated rivets is critical for strength. Contractor is found to have purchased sub grade steel but will have documents showing it meets spec. Inspectors will say that city officials signed off on the design and there was insufficient funding to trace the metal back to where it really came from. Mean time injured workers wil be tied up in court for years trying to sue for millions and in the end the lawyers will walk away with all the money.
