i did this about 2 years ago with a guy at work. if it is an even trade there is no tax. all i remember paying is a fee to transfer title.

i would say the dog tag is a good idea with a contact for emergency number. in an accident or emergency room setting medical info is only needed if you have a severe allergy(like pratt said). your emergency contact should be able to give medical info if you are unable.
