To make the gun look loaded, they drained the gunpower from some catridges and replaced the bullet. But the aforementioned idiot left the primer caps live. In a previous shoot, the trigger was pulled on one of these "dummy" cartridges and the cap was just enough to lodge the bullet in the barrell. Because the bullet was in the barrell, it was missed when the gun was cleared and a blank cartridge was loaded.
