  1. I think he was pissed when Meyer challenged that 1st down ruling on the field late in the game and got it overturned. 56-7, and Meyer still won't give him a break (which I think is hilarious). Me thinks Meyer was trolling him.
  2. I don't get what the hate is all about. They're not terrible looking, and the interior/features appear pretty nice for the price range. What am I missing?
  3. I've been thinking about leasing one to commute with for the next few years. I wouldn't need to put many miles on it, and have no real intentions of keeping it. Let me know how you like it.

  5. I came in expecting flash cards, however, am not disappoint. When can we put this into actual cars so I everyone can commute @ 100mph?
  6. Why not double time? A second job? Again, it's not my fault you choose to only put forth 40hrs/wk (of the 168 available) towards earning a living. Resident physicians are (theoretically) capped to around 80hr/week (88 for surgeons), though most put in well over that amount. It's even worse for Fellows. Given that, they still only earn an income of ~$50,000/yr; give or take. This is with $250,000+ of school loans hanging over their heads. Now, do the math on that and get back to me. And then tell me again how poor and oppressed those poor Walmart employees are. Back on topic, I think all those in Congress are blithering idiots, on both sides of the aisle. I'm hoping the general public takes this opportunity (they won't...) to oust both parties come November. And I hope that the ball keeps on rolling. This two party shit has got to end.

The world is full of choices, and it's not up to me to make the correct ones for you, nor is it my place to compensate for you when you make a bad one. Nobody is forcing you to work at Walmart. Nobody is forcing you to go through life uneducated. Nobody is forcing you to only work 1 job, or less than 40hrs a week. Get up off your lazy ass if your way of life is not sufficient to support your ways. Do not ask of me something you are perfectly capable of doing yourself. Now, for those that are not capable; come and talk with me. You'll find I have a different tone.

Let me give you some highlights from an email our hospital's CEO sent out to everyone today... -"Consistent with what is happening nationally and in central Ohio, 2013 has been a mixed year in terms of our financial results. Although the year started strongly, we have seen decreased patient volumes since May 2013. As you know, some of the current and upcoming healthcare changes are making it harder and harder to survive in today's market. Many of you have been directly involved in managing our resources to census and the changing healthcare environment. I appreciate the business acumen you have shown." -"What have we done to adapt to these changes? Looked hard at existing business lines. Closing inpatient rehab and the Walmart Acute Care Clinic are examples of suspending services that are no longer financially viable. Additionally, we chose to fully utilize the talents of existing leaders to manage critical areas such as Lab and ICU, rather than hiring additional employees." -"I want to thank all of you for the heavy lifting that has helped us meet many of our goals for 2013. Despite our best efforts, it has been a challenging year. Demand for healthcare services as well as reimbursement are down throughout the nation, region, and state. This has resulted in a number of large and small facilities announcing reductions in services and staffing." -"Our efforts and teamwork have helped us remain stable and have allowed us to move forward in spite of these challenges. Unfortunately, the realities of the business environment do impact us. Our business challenges prevent us from providing merit increases this year." (I'm OK with this; just pointing out another "benefit" of the ACA...) Next up: lay offs. Fuck you, Thorne. You don't see it because you're not involved in it. The healthcare sector is one of the biggest job sectors in the country. The ACA is destroying it from the inside.

  14. This stuff is already out there, in use. Our little rural hospital has a contract with Ohio Health for their TeleStroke network. Essentially, a patient presents to the ED with stroke-like symptoms. The staff fires up the Telestroke shit, where a neurologist from Ohio Health assesses the patient, and makes the call on what to do with them from there. It's pretty basic, but it's getting more and more involved. Our own ICU has talked about converting to an "e-ICU" in an attempt to save monitoring costs. What it amounts to is a VERY hi-def camera mounted on the wall closest to the foot of each ICU bed. The camera faces the wall until activated to avoid the feeling of being spied on. The hospital contracts with a monitoring company (I think they're in Cinci) to assess the patient at regular intervals in the hopes that they can catch something we don't, or before we do, as well as to cut back on our staff. The monitoring staff has access to all the same telemetry monitoring that we do and are supposed to sit there and monitor the patient 24/7. Should we need a doctor immediately, we would be able to speak directly with the monitoring company who has a Dr. right there as well. While I do not feel that it is a bad idea to have additional monitoring of a patient, I do not think it a good idea to replace physical, on-site staff with someone who's 150miles away. My mother, who's about to finish her MSN/MBA program, did a large project on telemedicine, and, like it or not, it's coming. I assume it's mostly driven by the desire to save costs, which isn't always a good thing. Everyone's going to have access to healthcare (!), but who knows how good that healthcare is going to be in the future.
