I lost track of the phone number of the buyer, and I do not have the PM anymore . You will also need to have or buy an accessory kit (spacers, hub nut, weights, etc). Which, if you can't find used locally or on Ebay, can be purchased from Hunter's website. It is 220v, and according to Scott (Buckeye who I bought it from), this balancer can do up to 22's. EDIT - This is AS-IS. I have never used this since I bought it from Buckeye.
