They are intensely painful, and by the look af that one, he had a bit of gangreen setting in (the dark lart on the left). This is not something that should be screwed with and in reality can be deadly (very rare but possible). Boils are similar to a zit being that they are pockets of infection, but zits will not work downward and into a bone causeing a bone infection.
