Sorry its covered in stuff on the left side of it, but its in good shape, and rarely used in my house other than a holder fo stuff in the spare room that we dont use. It was however used when ibought it, but in good shape.

Does the headboard and footboard require rails and are they included?

They will need rails, however their is a kit you can get from amazon that will mount the basic metal frames that come with most bed.
